WHO to Switch to One Dose of Two-Dose Cholera Vaccine Amid Rising Outbreaks

The World Health Organization said on Wednesday it will temporarily suspend the standard two-dose vaccination regimen for cholera, replacing it with a single dose due to vaccine shortages and rising outbreaks worldwide.

The U.N. agency said “the exceptional decision reflects the grave state of the cholera vaccine stockpile” at a time when countries like Haiti, Syria and Malawi are fighting large outbreaks of the deadly disease, which spreads through contact with contaminated water and food.

As of October 9, Haiti had confirmed 32 cases and 18 deaths from the disease, while many cases were still awaiting confirmation.

“The pivot in strategy will allow for the doses to be used in more countries, at a time of unprecedented rise in cholera outbreaks worldwide,” WHO said in a statement on Wednesday.

The WHO’s emergencies director Mike Ryan told reporters in a briefing that the change in strategy was a sign of the “scale of the crisis” caused by a lack of focus on safe sanitation and immunization for all at risk.

“It’s a sad day for us to have to go backwards,” he said.

The one-dose strategy had proved to be effective as a response to cholera outbreaks, the agency said, although the duration of protection is limited and appears to be much lower in children.

The disease often causes no or mild symptoms, but serious cases cause acute diarrhea and can kill within hours if untreated.

Cholera cases have surged this year, especially in places of poverty and conflict, with outbreaks reported in 29 countries and fatality rates rising sharply. The WHO also said that climate change means that cholera is a risk in an increasing number of countries, as the bacteria causing the illness multiplies faster in warmer waters.

A cholera outbreak in Syria has already killed at least 33 people, posing a danger across the frontlines of the country’s 11-year war and stirring fears in crowded camps for the displaced.

A cholera outbreak in a north Cameroon refugee camp has killed three people and infected at least 36, the UN refugee agency said on Wednesday.

The first case was confirmed on Saturday in the Minawao refugee camp, which hosts around 75,000 people who fled Boko Haram insurgents in neighboring Nigeria.

Malawi Finds Mass Grave of Suspected Ethiopian Migrants

Malawi has discovered a mass grave in the north of the country containing the remains of 25 people suspected to be migrants from Ethiopia, police said Wednesday.

“The grave was discovered late on Tuesday, but we cordoned it off and started exhuming today. So far, we have discovered 25 bodies,” police spokesman Peter Kalaya told AFP.

Police were alerted by villagers in the Mzimba area, about 250 kilometers north of the capital, Lilongwe, who stumbled onto the grave while collecting wild honey in a forest.

“We suspect that they were illegal migrants who were being transported to South Africa via Malawi,” he said.

He added that evidence gathered from the site indicated the victims were Ethiopian males ages 25 to 40 years.

The decomposing bodies were exhumed and taken to a morgue for autopsies.

The bodies appear to have been buried “probably not more than a month” ago, he said.

Malawi is a popular route for illegal immigrants from East Africa being smuggled to South Africa, the continent’s most industrialized country and a magnet for poor migrants from elsewhere on the continent.

Kalaya said that between January and September this year, authorities intercepted 221 illegal immigrants, 186 of whom were Ethiopians.

WFP Malawi Country Brief, September 2022

Strategic Outcome 1

• 2022/2023 Lean season response: WFP is working with the Government of Malawi and coordinating with other humanitarian and development partners to plan, jointly resource and respond to the 2022/2023 lean season needs. The Government of Malawi released its response plan for the upcoming lean season which requires USD 76 million for implementation of the response. Part of the requirements will be covered by other sources including the African Risk Capacity insurance pay-out worth USD 14.2 million as well as maize to be drawn down from the Government’s Strategic Grain Reserve, managed by the National Food Reserve Agency.

• Refugees: Monthly distributions were completed via cash (e-payments) for 11,000 refugee households (approximately 48,000 refugees) in Dzaleka refugee camp. WFP is preparing to conduct an updated targeting exercise in November to facilitate WFP’s refugee assistance.

• School feeding: The Deputy Minister of Health along with WFP Country Director visited WFP’s school feeding activities, Tsogolo la thanzi- TSOLATA supported by the European Union in Nsanje District. This visit was aimed at appreciating the linkages within TSOLATA, including smallholder farmers supplying food commodities to the schools and health promoters supporting volunteer parents to prepare nutritious meals among others.

• Social protection: In collaboration with the Government and partners, WFP has been providing capacity strengthening through technical assistance to enable effective leveraging of the social protection system.
After-action reviews of the 2021/2022 lean season and 2022 floods responses revealed successes in the use of the unified beneficiary registry for targeting during the lean season response. Furthermore, the finalised cash plus guidance (led by WFP) was highlighted as a successful pilot for beneficiaries.

• Malnutrition prevention: WFP has transitioned implementation of malnutrition sensitive interventions from cooperating partners to implementation through district councils to build Government capacity at local level.
